Data Request Instructions:
The Data Request Form is used to submit and prioritize requests for services provided by the Institutional Effectiveness and Assessment (IEA) office. This process helps our department manage workload efficiently while ensuring requests are completed accurately and on time.
When submitting a request, please provide as much detailed and specific information as possible. Include the purpose of the request, required data elements, desired format, and any deadlines. Attach any comparative or supplemental data if available. Please indicate whether it will be used for internal purposes, external reporting, or shared distribution. Additionally, identify the intended audience for the data (e.g., internal departments, executive leadership, external agencies).
Be sure to include any specific content requirements, such as:

Requested graphs, charts, or visualizations

Formatting or presentation preferences
The more detailed your submission, the better IEA can assess, prioritize, and accurately fulfill your request.

IEA is committed to ensuring the confidentiality of institutional data. Unauthorized use, access, or disclosure of any information or electronic file pertaining to the data request is strictly prohibited. IEA reserves the right to modify or deny a data request if it violates local, state, or federal laws.
