Effective Jan. 1, 2014, state law requires all new and transfer college students under the age of
22 to provide proof of bacterial meningitis vaccination. Students must provide proof 10 days before the start of the semester.
Students may file an affidavit or certificate signed by a licensed physician stating that the physician believes that the required vaccination would be injurious to the health and well-being of the student. The student may request an affidavit form from the Texas Department of State Health Services at
